I am now a Home Medicines Review (HMR) pharmacist, specialising in developing comprehensive medication profiles tailored to individual needs, with a strong focus on minimising medication-related harm and optimising treatment regimens. What is a Home Medicines Review?
For Patients
A Home Medicines Review (HMR) is a tailored, in-home assessment where I help you better understand and manage your medications. My goal is to simplify your regimen while reducing the risk of unwanted side effects or medication-related complications. During our visit, we’ll go through your medications together, discussing how they fit into your daily routine and addressing any concerns you may have. I’ll provide clear guidance to help you feel more confident about your treatment. After our conversation, I’ll prepare a detailed report for your doctor, outlining your current medications and any recommendations to optimise your healthcare. At your next appointment, you and your doctor can review the report, ensuring to make any changes that support your health and well-being.
For GP’s
A Home Medicines Review (HMR) is an in-home assessment conducted by an accredited pharmacist to optimise medication management, particularly for patients at risk of polypharmacy-related harm. The pharmacist evaluates medication appropriateness, adherence challenges, drug interactions, and deprescribing opportunities using evidence-based tools. A comprehensive HMR report is then provided to the referring GP, offering clinical insights to support informed decision-making and facilitate discussions that enhance therapeutic outcomes and continuity of care.
How Does A Home Medicines Review Work?
Doctor Completes a HMR Referral Form
A doctor determines if a HMR is needed and submits a referral to the corresponding accredited pharmacist.Pharmacist Consultation
An accredited pharmacist arranges a review at the patient’s home.Medication Assessment & Report
The pharmacist evaluates medications and provides a report with recommendations to the referring doctor.Review & Follow-up
The doctor discusses recommendations with the patient, making any necessary adjustments.
Eligibility
Patients must:
Hold a Medicare or DVA card
Live in a community setting (reside in their own home)
Be at risk of medication-related complications
Have a clinical need for an HMR as confirmed by their referring physician
Cost
Home Medicines Reviews are fully funded by the Australian Government for eligible citizens and residents.
Patients who hold a Medicare or Department of Veterans’ Affairs (DVA) card receive this service at no cost.
